All Care: If this is your first time with any of the discipline please book an Initial (discipline) Visit. If you are booking an appointment for your child(ren) please book it under their name. You will need to create a separate profile for your child(ren).

If you’ve had a new traumatic injury or have a new complaint please book an Initial (discipline) Visit.

We do NOT accept or bill insurance, Medicare, and/or any other third party carrier for payment (Ex. workers compensation or personal injury cases)

We may or may not see Medicare eligible patients. We are non-participating providers. Please call or come in to see us if we may treat you. The value of offers may differ for patients eligible for Medicare, and restrictions may apply.

Intake forms must be filled out prior to arriving to your scheduled appointment. Forms are normally generated when signing up for an appointment. Respective initial intake, informed consent, and other forms are sent to the email that was provided during registration.
